The CDM Adult Day Services Center is a place for socialization, rehabilitation, meaningful activities, and a sense of community. Our programs provide respite for caregivers, nursing and physical therapy services, and, most of all, a place for people to feel part of something. Our center specializes in purpose-driven activities, including volunteer projects for other non-profit agencies, which add to the sense of community.
The Center is located at 2409 Broadway St in Uptown Vancouver, one block south of Fourth Plain Boulevard. Hours are 7:30AM to 6PM Monday through Friday.
We provide services funded by Medicaid, Respite, the Family Caregiver Program, the Veteran's Administration, and other publically funded programs. We also accept most long-term care insurance, and have competitive private-pay rates with discounts available.
Some of the specialized programs that the Center offers, in addition to respite for caregivers:
Nursing Services – Licensed nurse provides medically necessary services including administration of medications, blood pressure monitoring, blood glucose monitoring, and diabetic foot care.
Restorative Therapy - Restorative services are skilled services that are overseen by a licensed Physical Therapist and administered by our restorative aid. An assessment is conducted and an individualized program is built by the physical therapist and reviewed every 90 days. This program is to maintain or improve balance, strength and mobility to reduce falls and maintain mobility independence.
"Total Recall": Total Recall is a program designed for persons of all ages with Traumatic Brain Injuries due to accident, trauma, or medical issues. Activities in this program are designed to exercise the brain to strengthen and build new neurological pathways. Working memory, long-term memory, and sensory memory are exercised daily. CDM Services also has a Traumatic Brain Injury Resource Coordinator on staff to help navigate the maze of resources available in the community.
"Journeys": Journeys is a program designed to help those with dementia or other functional decline. Through orientation and sensory activities, particpants access the creative side of the brain that remains intact during the process of dementia.
Caregiver Support Groups: Free caregiver support groups are offered at the Center by experienced caregivers who understand caregiving for a loved one with long-term care needs. "Creative Connections": Creative Connections is an arts-based program which seeks to connect to clients with dementia and other functional limitations through visual, performance, musical, and other artisitic endeavors. Please see the "Creative Connections" link to the left for more information on this new and innovative program.
"Paths to Mindfulness": Paths to Mindulness is proactive approach to memory loss, a three hour weekly class designed by Judy Canter MSW, LCSW of Mindful, Inc in which participants build their brains through activities, memory training techniques, and camaraderie.
